Applicants are required to appear in person and submit the following documents:
- Duly filled Life Certificate application form – to be submitted in duplicate.
- Valid original Indian passport / Canadian passport / OCI Card, as applicable.
- Photocopy of the first and last pages of the above-mentioned document(s).
Important Information:
- No fee is charged for issuance of Life Certificates for pension purposes.
- In case the applicant is unable to attend in person due to medical reasons, as an exception, the bearer of the application must set up a video call with the applicant for verification.
